All Ages

  • Twenty minute virtual evaluation for new patients via Zoom

  • Preliminary assessment to determine most appropriate plan of care

  • Cost of virtual evaluation will be credited toward the cost of an in-person Initial Evaluation or a Return to Sport Test

INDIVIDUAL FOLLOW-UP
All Ages

  • One hour in-person follow-up sessions for current patients

  • Routine follow-up sessions to monitor progress — whether you're working through an ongoing plan of care or following up after a Return to Sport Test

  • One hour evaluation for new patients

  • Designed for anyone managing an acute injury

  • Includes strength testing and any additional assessments deemed appropriate to establish a clear plan of care

TELEHEALTH FOLLOW-UP

  • 15min telehealth follow-up sessions for current patients via Zoom

RETURN TO SPORT TESTING
All Ages

  • 75min evaluation for both new and current patients

  • For athletes recovering from major injury or surgery

  • Identifies remaining deficits and performance gaps to determine readiness and what still needs to be addressed before returning to full activity.
